Patient Questionnaires (RCADS)

These questionnaires are about how you have been thinking, feeling and coping, how you process information and your family relationships. Your answers help us understand how things are for you now and how things have been for the last two weeks and what symptoms of anxiety or low mood you are currently experiencing. Please take time to read the questions carefully and answer all the questions. There are no right or wrong answers. We will keep your answers confidential, which means not telling or showing anyone, other than those who need to know. Your clinician will talk to you more about this.

There is one questionnaire to be completed by the young person and one for their parent or carer to complete.

Why Do I Need to Complete These Measures?

As a publicly funded organisation, we have a responsibility to demonstrate that the care we provide meets certain standards. This includes being able to demonstrate meaningful improvements in the health and wellbeing of our patients, following a period of care.

As an organisation, we are also committed to striving for excellence. This means that we are constantly trying to improve, to offer the best possible care for our patients. Of course, we are only able to do this if we know what our patients want and how we can do better. By sharing your views and experiences, you help us to offer the best possible care for you, your loved ones and your community.

How Will my Data be Used?

The data goes into your individual clinical record and used to help guide conversations around your care planning and individual aims.

If you would like to complete a survey around how we can improve your care you can access the “I Want Great Care” website. This survey is designed to assess your satisfaction with the care provided and give you the opportunity to suggest areas for improvement.
